Health Counseling Introductory Session

FREE 15 minute session that allows us to get to know one another to determine if we are a good client/practitioner fit. Please share with me your most pressing health and nutrition concerns. This may be done over the phone or in my office.

Health Counseling Discovery & Education Session

Prior to this 90 minute meeting, you will provide me with thorough intake forms outlining your health history, health concerns and general eating habits. I will individualize the session to your specific needs and provide you with educational materials and practical tools in order for you to make successful, long-term lifestyle modifications. I will encourage you to make 3 health-related goals that we can use to track your progress. This meeting is all about you! At the end of this session and each subsequent follow-up session, we will discuss whether you require 30 or 60 minute follow-up sessions depending upon your individual needs. You must meet with me for a 90 minute Discovery & Education Session prior to scheduling follow-up appointments.

Health Counseling Follow-up Sessions

This 30 or 60 minute meeting will allow us to assess the changes you have made and determine your level of success in meeting your health goals. Our bodies are in a constant state of flow so we will modify and/or add more goals as the need arises.

Health Counseling Vitality Package: 5 Sessions

This package includes the 90 minute Discovery & Education Session in addition to 4, one hour follow-up sessions and a grocery shopping tour for a $25 savings!!! This package is ideal for couples, families, people struggling with 3 or more nutrition related health issues or for those who need extra guidance and support on their journey to health.

Grocery Shopping Tour

Feeling confused about where to shop and how to shop now that you are trying to eat healthier? I will meet you at your preferred grocery store and guide you in making the best choices that fit your budget. No prior appointments are required to utilize this one hour service. However, if we have already met, I will have helped you create a shopping list; if this is our first meeting, please come up with a general list of foods you would like to purchase. 

Health Counseling Home Visit

This service consists of a two hour visit in your home where we have a Discovery & Education Session and you may allow me to peek into your pantry and refrigerator in order for me to gain in depth insight into the way you and your family live and eat. No prior appointments are required to utilize this service. If we have already met in the office, this will be a one hour follow-up visit to help me better understand the way you and your family live and eat.

*Follow-Up Sessions + $15 ($100/hour or $70/half hour total)
